Woburn Care Company is a privately owned domiciliary company that provides high quality Personal Care and support at your own home. We actively promote independence and understand the importance of detail when it comes to assessing your needs and wishes. Your Care Plan will be tailored to your needs and are personal to you as an individual. All our Staff is DBS checked and undergo mandatory training and an induction course, further development is through education and training.

History
Your care plan will be discussed with you in length and any family member/representative should you wish. We will discuss you're wishes likes and dislikes in relation to you're care.
We undertake a thorough assessment, which includes a social and physical profile, risk assessments for medication, moving and handling and environmental. This allows us to build a personalised care plan. We understand that you're needs may change at any given time and you're care plan will be individual with the flexibility to change should you want to.
The lengths of visits will be tailored to meet you're needs and can be discussed at the time of assessment.

Chelsea Monteiro
Mental Health Officer
Chelsea started her care career initially supporting children with special needs education in schools.
She is very into ensuring people are happy with the care they receive and ensuring quick and effective solutions to our service users and families.
Chelsea has experience working with people within the Mental Health side and people with different health conditions such as Diabetic service users, people suffering from Dementia, cancer and neurological health conditions affecting mobility.
She is currently completing her Level 5 in Healthcare Management and always ensures that staff feel valued and welcomed in the service.
Chelsea is our current Mental Health Officer and provides counselling services to our staff.
